Primary Care
蹤獲扦 internal medicine physicians provide comprehensive primary care for adults of all ages and adolescents beginning at age 16. Outpatient services are offered at locations throughout Jacksonville. Physicians also see patients admitted to the medicine teams at 蹤獲扦 Jacksonville.
We also offer two unique services to the Jacksonville community:
- Geriatric and hospice and palliative care services are offered, including a comprehensive geriatric assessment clinic, a dementia clinic, as well as inpatient hospice and palliative care consultation to patients.
- The Program for Adults with Intellectual and Developmental Disabilities (PAIDD) provides primary care and a medical home for adults with intellectual and developmental disabilities.
