Description
This study is multifaceted and has multiple aims.
1. Discover what role religiosity and spirituality play in anticoagulant (AC) use in non-white older adults with atrial fibrillation.
2. Utilize the PPC model to improve the engagement in shared decision making for stroke prevention in older adults with atrial fibrillation and multimorbidity, to ensure that clinical decisions are guided by the patient’s own priorities and health goals.

Eligibility
All patients will have atrial fibrillation.
Aim5 will be subsetted to the Age 65+.
Aim 6 will be subsetted to the Age 65+ who, in the past year, are community dwelling older adults with at least 2+ comorbid conditions and have had 2+ health care visits or 1+ emergency room visit, or hospitalization.
